보청기 급여제도

급여 지급 대상

보청기 1대 지원 대상자 * 다음 중 한 가지 조건 충족
중증
  • 2급. 양측 난청이 각각 90dB 이상인 사람
  • 3급. 양측 난청이 각각 80dB 이상인 사람
경증
  • 4급-1호. 양측 난청이 각각 70dB 이상인 사람
  • 4급-2호. 두 귀에 들리는 보통 말소리의 최대 명료도가 50% 이하인 사람
  • 5급. 양측 난청이 각각 60dB 이상인 사람
  • 6급. 편측 난청이 80dB 이상인 사람, 다른 귀의 청력 손실이 40dB 이상인 사람
보청기 2대 지원 대상자 * 다음 조건 모두 충족

아래 조건을 모두 충족해야 함.

  • 가. 19세 미만의 청각장애인
    (처방전 발행일 21.07.01 이후)
  • 나. 양측 80dB 미만의 난청 환자
  • 다. 양측 어음명료도가 50% 이상
  • 라. 양측 순음청력역치 차이가 15dB 이하
  • 마. 양측 어음명료도 차이가 20% 이하

급여 지급 기준

보장구 지원 혜택은 5년에 1회 진행되며, 최대 지원 가능한 금액은 131만원입니다.

급여 신청 절차

▶ 청각 장애 등록 절차

Tip 급여 신청을 위해서는 우선, 청각 장애 등록이
되어 있어야 합니다.

    1단계 진단 의뢰

    • 장애진단의뢰서 발급

  • 의료기관 / 병원

    2단계 장애 검사

    • 청력검사 실시
    • 청각장애 진단서 발급

  • 주민센터 (동사무소)

    3단계 서류 제출

    • 장애 진단서, 증명사진 (2매)
      검사 결과지, 진료 기록지 제출

  • 국민건강보험공단

    4단계 심사 요청

    • 장애 등급 심사

  • 주민센터 (동사무소)

    5단계 복지 카드 발급

    • 장애등록 및 복지카드 발급

▶ 급여 신청 절차

  • 의료기관 / 병원

    1단계 보청기 처방

    • 이비인후과 전문의로부터
      보청기 처방전 발급

  • 보청기 센터

    2단계 보청기 구입

    • 유니트론 보청기 구입
    • 보청기 구매 표준 계약서 작성
    • 보청기 구매 영수증 발급

    기초생활수급자는 구청 또는 주민센터에 처방전을 제출하고 적합통지 후, 구매 영수증 발급

  • 의료기관 / 병원

    3단계 검수 확인

    • 보청기 착용 개선 효과 확인을 위해
      검수 확인서 발급

  • 국민건강보험공단

    4단계 급여비 청구

    • 급여비 청구를 위한 관련 서류 제출
      • 1) 복지카드
      • 2) 보조기기 처방전
      • 3) 보조기기 검수확인서
      • 4) 급여비 지급청구서
      • 5) 세금계산서 또는 영수증
      • 6) 바코드 부착사진
      • 7) 통장 사본

  • 보청기 센터

    5단계 후기적합관리

    • 구매 1년 후부터 매년 1회 총 4회
      후기 적합관리급여 신청
      • 1) 보청기 적합관리 급여 청구서
      • 2) 청력측정 결과지
      • 3) 데이터 로그 기록지 (피팅 리포트)
      • 4) 세금계산서 또는 영수증
